These data collection worksheets can be used for a range of different open-ended data collection lessons.

A variety of different templates are included.

LEVEL 1:

  • Collect data for 3, 4, 5 or 6 different answers

  • Record which answer had the most and least votes

LEVEL 2:

  • Collect data for 3, 4, 5 or 6 different answers

  • Record which answer had the most and least votes, as well as an additional observatino about the data

Possible way to use these templates:

Explain that students are  going to create their own question with 4 answer options. Have a quick brainstorm of possible topics with the class for students who aren’t sure (favourite: superhero, movie, pizza topping)

Instruct students to record their question, then ask 10 classmates and record the answers in pictures or lines next to each answer box. Students can then answer the questions:

  • Which answer had the LEAST votes?

  • Which answer had the MOST votes?
